Interim results from 500 patients in the trial, called Impact, are expected in October. In a conference call this afternoon, Dendreon announced its second-quarter 2008 financial stats, as well as giving a brief update on two other, smaller clinical trials of Provenge. “We’re continuing to make progress on multiple fronts, including our pipeline,” said CEO Mitchell Gold. “Our foremost priority remains advancing Provenge through the approval process.” Indeed, if the drug goes on to gain FDA approval and takes off, the commercialization plan calls for building a sales team in the U.S. and an outside partnership to distribute the drug abroad, said Gold. Dendreon has just begun enrolling patients in its NeoACT trial, which will involve 40 patients at the University of California, San Francisco, with localized prostate cancer who are scheduled to undergo a prostatectomy. The clinical trial “provides us with an opportunity to look directly at the organ, the prostate, and its immunotherapy response,” said David Urdal, Dendreon’s chief scientific officer. Another clinical trial of Provenge, called ProACT, is expected to begin later this month. It is a multi-center trial involving about 120 patients with metastatic prostate cancer. The goal, said Dendreon, is to gain new scientific insight into Provenge, and gauge its impact on patients. In a statement, Dendreon (NASDAQ: DNDN) reported a Q2 loss of $16.5 million (18 cents per share), and its quarterly revenue dropped to $26,000 from $523,000 in Q2 of 2007. The company said it holds $127.3 million in cash, cash equivalents, and investments, including $46 million raised in a stock offering in April. Biotech, Provenge, Dendreon Luke Timmerman wrote: Dendreon’s David Urdal never fails to give off the impression that he’s a ballast for the company, at least in the dozens of times I’ve interviewed him in the past seven years. The even-keeled chief scientific officer has lived through all the ups and downs since he joined the Seattle biotech company back in its early days in 1995. Now Urdal, 58, is gearing up for another turning point in October. It’s from a clinical trial of 500 prostate cancer patients, known as Impact. The company (NASDAQ: DNDN) will find out if that trial can crank out the kind of evidence needed to convince the FDA it truly has a drug that can help prostate cancer patients live longer. If it does, the company will sprint to bring that drug, Provenge, to the market as the first treatment of its kind to stimulate the immune system to fight cancer cells. But if the early peek at the data—an interim analysis—shows results that could even possibly be due to chance, then everyone will have to wait in suspense one more year for a final verdict. “I think people here now can feel the moment coming,” Urdal says. “We’re within a couple of months of knowing new information, and there’s a high level of anticipation and excitement about it.” Provenge has captivated the imaginations of doctors, patients, and investors (and journalists) for years. The treatment, called an immunotherapy, or a cancer vaccine, doesn’t work like a traditional cancer drug. Blood is drawn from a patient, and some white blood cells vital to the immune system, called dendritic cells, are separated in a lab. The cells are shipped to the company and incubated with a genetically engineered protein found on prostate cancer cells, called PAP. The white blood cells are supposed to recognize the protein as an invader and recruit the immune system’s arsenal to attack tumor cells that contain it. The revved-up white blood cells are shipped back and re-infused into the patient. Without going too long on the history—that’s a book-length project—Dendreon thought it had done enough to get Provenge approved by the FDA a year ago. It asked the agency to clear the product for the sale mainly on a study of 127 men that showed it could extend patients’ lives by a median time of 4.5 months, with minimal side effects, like fever and chills. An FDA advisory panel agreed in March 2007 that the drug was safe and “substantially effective.” The next day, Dendreon became the heaviest traded stock on the entire Nasdaq exchange, rocketing from $5.22 a share to briefly more than $20 after the panel vote, as investors were betting that the FDA would follow the panel’s advice and approve the drug, as it usually does. The operative word in that last sentence is “briefly.” Dendreon crashed back to earth in May of last year. That’s when the FDA chose not to approve the drug, preferring to wait for more evidence from the ongoing 500-patient Impact trial. Almost $1 billion of stock market value evaporated in a heartbeat. He’s building a startup, called North Coast Biologics, around the idea—without a penny from venture capitalists or more than a handful of employees. Ridiculous, right? “I wouldn’t bet against him,” says Carl Weissman, the president of Accelerator, the Seattle biotech-startup incubator. Accelerator invested in Stine’s previous company, Spaltudaq, which has now raised $34 million in venture capital to develop antibody drugs. To understand Stine’s new idea, a bit of background is necessary: Genetically engineered antibody drugs, which can zero in on diseased cells while sparing healthy ones, are the fastest-growing class of drugs in the pharmaceutical industry, expected to top $26 billion in global sales by 2010. South San Francisco-based Genentech (NYSE: DNA) has become the world’s most valuable biotech company on the strength of three such drugs for cancer: Avastin, Rituxan and Herceptin. But those medicines all have origins in a technique discovered in 1975, using mouse cells. The immune system recognized them as foreign and rejected the drugs, so companies like Genentech used less mouse DNA, and incorporated more human DNA, into their antibodies. There are problems, however, with using mice, Stine says. They don’t make antibodies against some proteins that humans consider foreign, while rabbits do, he says. Rabbit antibodies have 1,000 times higher “affinity” than mice (they bind with their target on cells much more tightly and for longer). For the biotech business, that means rabbit-generated antibodies can be given in fewer shots, and at much lower doses, saving a bundle on manufacturing costs. And here’s the part that Stine says “blows people away.” With rabbits, he has developed a way to yield hundreds of antibody drug candidates in less than a month. By comparison, it can take several months for mouse methods to yield a single drug candidate, he says. He calls it BLAST (B-Lymphoblast Activation and Selection Technology). BG Medicine, a privately held diagnostics maker in Waltham, MA, sees opportunity in saying when that’s a waste of time and money. Take, for example, Genentech’s Avastin. The drug, the first of its kind to choke off blood supply to lung, colon, and breast tumors, is one of the world’s best selling cancer medicines, with $2.3 billion in U.S. sales last year. It costs about $4,400 a month in a low dosage, and $8,800 for a higher dose for lung cancer patients. Yet studies listed in the drug’s prescribing information show it’s effective in less than half of patients, and many people who respond initially later develop resistance. No surprise, health insurers want tests that can sort out who will benefit from such pricey drugs, and who won’t, says Pieter Muntendam, BG Medicine’s CEO. They apparently want them badly enough that they’re willing to pay high prices for tools to do it. One company, Genomic Health (NASDAQ: GHDX) of Redwood City, CA, has had success marketing a genetic test—for more than $3,400—that predicts whether an early-stage breast cancer patient is likely to relapse, and whether it’s worthwhile to take preventive chemotherapy. The market for such “molecular diagnostics” is expected to balloon from $18 billion in 2006 to $92 billion in 2016, according to Kalorama Information, a market research firm. “The big opportunity is in better matching patients with treatments,” says Muntendam. “It’s hard to convince payers to pay $100,000 for a drug that works for 15 percent of patients.” BG Medicine, for its part, envisions developing some low-price, high-volume tests for $20 to $150 apiece, and others that will cost $1,500 to $2,000, Muntendam says. The company just raised new capital this month to make it happen. BG scrapped plans to do an initial public offering earlier this year, and instead pulled in $40 million from a group of venture capitalists to help commercialize its first product and continue work to discover new biological markers of disease, or predict response to medication. The group includes Legg Mason Capital, GE Asset Management, Humana, and Flagship Ventures. BG Medicine was founded in 2000 as Beyond Genomics. It got its legs initially from doing tests for big drug companies who wanted to know early on which patients might suffer liver damage in clinical trials, so they could be weeded out, giving clinical trials a better chance at success. The effort attracted plenty of big name partners, including Pfizer (NYSE: PFE), Merck, (NYSE: MRK) and AstraZeneca (NYSE: AZN). Now BG sees its future more in marketing its own products, says Noubar Afeyan, CEO of Flagship Ventures, and executive chairman of BG Medicine. First up, the company plans to ask the FDA later this year to approve its diagnostic that spots high amounts of a protein in the blood called galectin-3. That protein is supposed to predict whether patients will die of congestive heart failure within 60 days. Another test in the works looks for a protein in the blood that’s a sign of inflammation, which is thought to make the difference between fatty buildups sticking to the lining of artery walls, or busting loose and causing a heart attack, Muntendam says. The desire on the part of insurers to control costs will continue to support the work of diagnostics companies, Afeyan says. “This notion that the health care system can offer a new drug that doesn’t really do a better job of treating disease is highly dubious. So what do you do? You identify responders from non-responders.” Drug companies aren’t really resisting the change, which is already being pushed hard by the FDA in clinical trials, Afeyan says. Thousand Oaks, CA-based Amgen (NASDAQ: AMGN) has been touting data that shows its colorectal cancer drug Vectibix works for patients with normal KRAS genes, that control cell division, but doesn’t help patients with mutated forms, or an estimated 40 percent of people. Successful diagnostics like Genomic Health and Biosite (now a unit of Inverness Medical Innovations (AMEX: IMA)) have proven to venture capitalists that there’s a lot more money than there used to be in diagnostics. “In the last 12 to 18 months, you’ve seen more exits at higher prices in diagnostics than you did in the prior 10 years combined,” Afeyan says. “Diagnostics are probably the hottest sector of all in life sciences.” If BG can get its first products on the market, and convince doctors and insurers they represent a better way to practice medicine, then maybe BG will add its name to the list of successful ‘exits,’ that Afeyan is talking about. The story, as told by Caliper CEO Kevin Hrusovsky, goes something like this: Sugen, a small Redwood City, CA-based biotech company later acquired in 1999 by Pfizer (NYSE: PFE), was doing tests of an experimental drug in mice. Tumors of mice on the drug actually got larger than those in a control group. It didn’t make sense, because the drug, now called Sutent, was designed to block tumor proliferation and cut off blood supply. So researchers ran another test, with an optical imaging tool made by Caliper. The tool, which lets researchers see certain genes glowing in mice when they are being switched on or off, showed Sutent was actually doing what it was supposed to do. When the researchers looked more closely at the original result, the tumors still appeared bigger, but it was misleading, because they were filling up with water. With that knowledge, the researchers moved ahead with further tests. The drug won FDA approval in January 2006 for gastrointestinal tumors and kidney cancer, and generated $581 million in sales in 2007. Caliper’s sales reps lean on testimonials like that, because despite big research budgets, pharmaceutical companies are pretty conservative customers. “A lot of pharmaceutical companies won’t buy a product like this until it (the product) has a proven track record of bringing a drug to market,” says Hrusovsky. Caliper, like most companies that make picks and shovels for the gold-seeking biotechs, tends to keep a low profile. The company obtained the optical imaging tool when it acquired Alameda, CA-based Xenogen two years ago. It is now Caliper’s fastest-growing product line, with 30 percent annual growth, Hrusovsky says. Caliper expects to generate more than $140 million in total revenue this year, and to turn an operating profit in 2009. (It’s an operating profit, but not a profit according to generally accepted accounting principles, because Caliper has to take accounting write-offs for acquisitions of money-losing companies.) The company, which has about 250 of its 500 employees in Massachusetts, has been trying to fully exploit the commercial potential of the optical imaging business. Under Xenogen, the imager was sold mostly to academic researchers, Hrusovsky said, but Caliper is pushing it hard with large pharmaceutical companies. The devices aren’t cheap, at a cost of $150,000 or $350,000 for two different models, yet Novartis has bought 15, Merck has 10, and Amgen and Sanofi-Aventis have four each, he says. Cambridge, MA-based Vertex Pharmaceuticals (NASDAQ: VRTX), Wyeth, and Johnson & Johnson use it too. Caliper’s stock has been sliding much of the year, but has rebounded 53 percent since June 30, when it was added to the Russell 3000 Index. That meant Caliper shares were suddenly getting bought by mutual funds and index funds, Hrusovsky said. So where is this company headed? “We think we can get revenue close to $200 million in 2010,” Hrusovsky says. It is the level where the spiritual content of a person emerges. That which is the true essence. We are all here for evolving in consciousness and if the inner tools of obtaining a higher consciousness are broken or disturbed then the whole purpose of evolution in consciousness is lost. Then what is the meaning of life? An absolute healthy mind is expressed by a person living in a continual spiritual bliss. Daily revealing new creative ideas… Expressed in very clear ways, always in the service of others. That is health. I want to see that when your child experiences an earache, he or she is given natural remedies to help heal. Not antibiotics. And true understanding of the biological conflict that causes that symptom. I want to see that an allergy is cleared naturally, instead of being “treated” or suppressed with Claritin or whatever else is available in the pharmacy. Antibiotics, Pain killers, allergy medications, antacids and laxatives are all used and well over abused for the wrong reasons. I found this snip in an ancient text of Sumeria called “The Sacred Script of the Covenant“: “Honor your body, which is your representative in this universe. It’s magnificence is no accident. It is the framework through which your work must come true, through which both the spirit and “the spirit within the spirit” speak. The flesh and the spirit are two phases of your actuality in space and time. Who ignores one, falls apart in shambles. It is a system of medicine founded by Dr Samuel Hahnemann, who lived between 1755 and 1843 in Germany. It is based on a new principal that “like cures like”. What does this mean? A medicine capable of producing certain effects when taken by a healthy human being is capable of curing any illness that displays similar effect… This is known as the “law of Similimum.” A painting of Dr. Samuel Hahnemann - founder of Homeopathy - observing the horrors of “Allopathic Medicine” as it bludgeons and poisons patients to death - even for profit*: (*Note in the US, medical doctors are currently forced to sign contracts with HMO’s guaranteeing patients will not be told of new and better treatments and methods. Thus preserving HMO profits. That’s on top of one of the what is already one of the worst systems of health care per dollar spent. In the area of Cancer alone, new treatments have been received with SWAT Teams, attack dogs, lab burnings, and over 300 cancer-research scientists “dead” over the past 5 years - all under suspicious circumstances… the profits of this industry are enormous - and it stops at nothing to protect them. Cancer books are routinely forced out of circulation by the US Government - without regard to “freedom of the press” or constitutional rights…This is why it’s a big area folks. Better information is often fought savagely, to protect profits. In fact, it’s hard to find good health information on the Internet…a sad comment on the state of democracy, and ethics, but let’s move on with the article…) For example, if a healthy person takes a dose of arsenic, he will develop vomiting, diarrhea, a rapid pulse and prostration. His skin will become cold, and he will grow very anxious. If he continues using arsenic, his nose will be running, he will have a heavy head, cough, and bronchial catarrh. Later he will develop even more specific disturbances of the skin and nervous system. He will have a burning sensation all over, which is relieved by warmth. He will have frequent thirst for sips of water; he will have fear of death, restlessness, and the worsening of symptoms at noon and midnight. Now if any of these symptoms are experienced by a sick person, they can be treated by Arsenicum…. Because you can see that the original substance has attacked the physical level (runny nose, catarrh, cough, etc.) The emotional level (restlessness) and the mental level (fear of death, and prostration) Now - follow me carefully here - because this part is hard for most people to understand: So what is “Arsenicum”. It is arsenic diluted in a special way, take one part arsenic to 99 parts water and mix and shake well ( succussing is extremely rigorous shaking ) … Then take one part of this mixture and mix it again with 99 parts of water - and repeat… This is called dilution and potentization. When you repeat this process 12 times you have *not one atom* of the original substance left in the new mixture. According to the law/constant of Avogadro: To give you some idea how small these doses are - consider a concept of chemistry known as Avagadro’s number: This number is an extremely large number that becomes a negative number, it’s “602″ with twenty one 0’s after that! That represents the number of particles present in any given quantity of substance. It is also known as a “mole”. When you dilute more than 12 times there can’t be any atom left of the original substance. Potencies used commonly today, are 6C, 30C, 200C, 1000C, 10000C and 50000C. EXAMPLE: To give the reader some idea of how extremely diluted, let’s take for example a dilution of 100000C. Let’s describe it in terms of a numerical fraction. Avogadro’s number would correspond roughly to a dilution represented by 1/1000… to a total of 24 zeroes. A potency of 100000C would be represented by a dilution of 1/100000…to a total of 100,000 ZEROES! Inconceivably far beyond the point at which not one atom of original substance is left. Therefore, it has been said that “poison is the remedy for poison…” Hahnemann postulated this principle as “Similia Similibus Curentur” meaning roughly “like is cured by likes.” Ultrahomeopathy is the only medicine where the remedies are tested on human volunteers, or “provers”, to illicit the symptoms they will produce. So now understand that any medicine you put in your body, ie. “Tylenol, Aspirin, Zantac” etc which you take for a symptom, will cause other symptoms…These other symptoms are called by pharmaceutical companies “side effects”. They are not side effects - they are primary effects of the drug quite purposely misunderstood by a for-profit industry. They say “side effects”, because these are the effects that we are not interested in seeing, but everybody who takes that individual drug experiences a multitude of symptoms. It will cause symptoms on the physical level, emotional level, and mental level - even if you just take it for the physical level…depending on your susceptibility level to that substance. EXAMPLE: Taking Tylenol for a headache will come with maybe a bit of nausea, sleepiness, and often constipation. Which is the primary effect? And which is the secondary effect? To look at it accurately - They are all primary effects. So take for example common side effects of pharmaceutical drugs, headache, nausea, constipation, dizziness, death. This has become the number one cause of death in North America - right now. …Depending on how susceptible you are to a substance that is how you will react. The individuals who are extremely susceptible to a drug can have a real anaphylactic shock from something as simple as aspirin. What happened to the Hippocratic Oath of “above all, do no harm…”? …As in the 3rd law of Newtonian physics, “for every action, there is an equal and opposite reaction?” Allopathic medicine ignores this principle. How To Improve Sleep or Insomnia With Homeopathy EXAMPLE: Let’s take insomnia. We take a sleeping pill for that, so if you can’t sleep you take a pill that will knock you out. You sleep. This is the “action” of the pill, but the “re-action” of your body to that pill is wakefulness - as anybody who has taken a sleeping pill knows. In general “the next night” you have even more problems sleeping, you take one more pill - which won’t knock you off as hard as the 1st one - and in time the affect of the pill will get less and less and you will need stronger and stronger pills - or more and different kinds of pills. …This is the approach to sleeplessness coming from an Allopathic point of view… they are trying to set forth an action and trying to ignore the equal and opposite reaction. Over a lengthy period of time, you will definitely get increased sleeplessness. The homeopathic approach to the -same symptom- using the earlier “like cures like” - gives a remedy to increase the symptom - a remedy that in a sleepy person would create sleeplessness. Let me explain, by working with the “Law of Similars” the Ultrahomeopath uses the principle of equal and opposite reaction to bring healing. Giving the patient the correct remedy causes sleeplessness - which is an action. The system meets this with an equal and opposite reaction - which is sleepiness. Together - both reactions harmonize into a healing reaction. Ultrahomeopathy is a reactive medicine, one that places the healing impact on the equal and opposite reaction rather than an active medicine, such as Allopathy, that places the forces of cure on the first medicine action, and calls the equally potent reaction, “a side effect.” Can Coffee Improve Sleep? EXAMPLE: A potential remedy for sleeplessness in a good Ultrahomeopathy case-taking is “Coffea” - or coffee in diluted and sucussed or potentized form. Do you understand these laws at play now? If not, please go back and reread this section until it “sinks in”. When you “get it” - then you’ll understand how powerful this is. There are other remedies used for sleeplessness, you have to use the correct remedy that fits the picture of a person on the physical, and emotional and mental level - at the same time. Some practitioners do not understand this principle, so they feel like, that homeopathic remedies can be used like allopathic drugs - which couldn’t be further from the truth. ie. The watered-down, garden-style variety of “health food store homeopathy” - with the thinking that “This remedy is for headache, this remedy is for trauma, this remedy is for colds, this remedy is for allergies.” We’ll have further articles in this area - but this point is key. Improperly used homeopathic remedies can be as damaging as Allopathic drugs. Don’t play games with this. Self-prescribing is sure to hurt you. Example: Take a health food store or beginner homeopath “combination” or “cocktail” of remedies for your asthma - and all 9 will work! 1 for you, the other 8 against you. Get it? At first it might seem “that it worked” as in Allopathy, and later you will have a bigger, future disease to pay… In Ultrahomeopathy, we look at what is different, what makes each person unique, so we can treat the patient with individual precision…. Allopathic medicine looks for what is the same between patients and then applies a uniform treatment to the disease. When I get a patient and they say they are experiencing depression, I always ask for more information - what else are they experiencing? They usually experience a multitude of -simultaneous- symptoms like indigestion, head ache, and some menopausal symptoms. All the symptoms should be considered by the Ultrahomeopathic physician and instead of giving one “canned” remedy for depression, asthma, or headache - we have to treat the whole person with only 1 remedy, of an available 10,000 - instead of guessing and giving a remedy. We treat the whole person. It is not easy but that is what Ultrahomeopathy is. It’s a process of intricate analysis that has hyper-precise results….And takes years of training to accomplish. EXAMPLE: Now think of a multi billion dollar pain killer industry, think of a head ache. IF you take a painkiller for a head ache, does that actually cure that head ache? Of course not. There are doctors in about 30 countries testing his company’s experimental drug/device combo treatment in clinical trials of cancer patients. He meets the physicians in person, twice a year, to talk about the product and learn how it’s being used. Then there are always meetings to talk about it with investors and potential partners. He visits the company office only once or twice a quarter, he says. “How could I go into those meetings with investors if I didn’t do that?” Keltner says. “It’s invaluable experience.” Still, I was able to book an hour this week to meet the highly-mobile Keltner at his company’s Bellevue, WA headquarters, a week after he raised $40.1 million in venture capital. Most of what’s been written about the company has focused on the “it-failed-to-go-public” angle, yet it turns out Light Sciences Oncology has a much more important news event coming up. After 13 years in business, it will find out in a few months whether the company’s light-infusion technique can actually help cancer patients live longer. It’s been a long time coming for a company supported for years by Craig Watjen, the treasurer from the early days of Microsoft, and more recently, an array of venture capital firms that includes Essex Woodlands Health Ventures, Johnson & Johnson Development Corporation and Novo A/S. The market they are pursuing initially, liver cancer, affects an estimated 19,000 people in the U.S. each year, although the technology could be able to be applied to many more tumor types than that, Keltner says. To understand why, some background is required. The company’s product, called LitX, is attempting to become the first treatment of its kind. The system works by using a light-emitting diode, guided by a biopsy needle, which doctors can thread inside a solid tumor with the help of an ultrasound or CT imaging machine. The patient is then injected with an inactive chemical drug called talporfin sodium. Once the light is turned on, powered by cheap AAA batteries, it activates the drug to kill tumors within a limited wavelength, sparing nearby healthy tissue. The light stays on for almost three hours while the patient can watches TV or reads a magazine, and then goes home. The company has passed some early clinical trials that show tumor shrinkage, yet has said little about it, Keltner says, because he doesn’t want to raise false hope among patients about a treatment that’s not yet available. Now, he feels he can “loosen up a bit,” because LitX has advanced to the final proving ground, a Phase III trial of liver cancer, which could provide evidence needed to bring the product to market. The company began recruiting 200 patients for the test of LitX against liver cancer in November 2006.
