This is actually an air horn imitation I bought from walgreens. The user blows into a pipe which splits in two parts, each of which actuate a rubber skin stretched over a plastic tube. The two tubes are set an attention-getting dissonant interval apart. I’m not sure I ever play it normally during this piece though. Less
